     1        left of the heading "Crew information form", somebody has
     2        written in handwriting:  "Orientation at 10 o'clock
     3        Tuesday, 9th" which we know to be Tuesday, 9th November
     4        1993.  Do you know whose handwriting that is?
     5        A.  That is mine.

     7   Q.   That is yours.  There is a signature; do you see the box
     8        bottom left-hand corner "Management use only"?
     9        A.  Yes.

    11   Q.   There are two signatures at the bottom of that box,
    12        signature one, whose is that?
    13        A.  That is mine.

    15   Q.   That is yours.  We notice, we may notice in passing, that
    16        in the box beside that, the last of the big, long
    17        horizontal boxes, his salary in his previous job was £3 an
    18        hour, according to him.  Whose is the other writing on this
    19        page -- anybody's?
    20        A.  In the "management use only", that is my writing, and
    21        also the rest is Alan's.

    23   Q.   That is Mr. Beech's writing?
    24        A.  Yes.

    26   Q.   Would you be there when he was filling in this form if he
    27        filled in this form?
    28        A.  No, he would have handed that in.

    30   Q.   We notice also that in the third box up from the bottom of
    31        the long boxes he is asked the question:  "Can you work
    32        extra hours during school/college holidays?" and he says:
    33        "Yes".

    37   MR. RAMPTON:  Sorry, that is in the third box up from the bottom
    38        of the long boxes towards the bottom of that box:  "Can you
    39        work extra hours during school/college holidays?"  On the
    40        next page, Miss Moffatt, which is headed "Interview
    41        summary", there is a box with ticks, appearance,
    42        personality, availability and ease of travel which appears
    43        to be graded from plus to minus.  Who puts in those ticks?
    44        A.  I do.

    46   Q.   It says:  "Remarks:  Good personality, likes working with
    47        people own age and fast, active work", and then is that
    48        your signature again?
    49        A.  Yes

    55        A.  Yes.  Basically, you fill that out at the end of the
    56        interview, just some keys points that they mention; they
    57        say:  "I enjoy working with people", you put that down, and
    58        just a couple of key points.
